With the exception of surgeries requiring skin excision, most surgeries are performed through incisions made through the conjunctiva (mucous membrane behind the eyelid), avoiding skin wounds as much as possible.
Since postoperative pain and swelling can be minimized, we mainly perform day surgery.

With age, lower eyelid fat protrudes forward. This causes sagging and puffiness to appear in the lower eyelid.
Surgery is recommended for lower eyelid wrinkles. The incision is made according to the patient's eye shape, so skin scars are very discreet.
Dark circles in the eyes are caused by sagging skin and fatty deposits that accompany aging. However, these are not the only causes of dark circles. On the anterior surface of the bone under the eyes, there is a ligament called the orbicularis retaining ligament, which connects the skin to the bone.
The increase in fat not only causes the fat to protrude, but also causes a concavity because this ligament just below the fat pulls hard on the skin, creating a shadow due to the gap between the bulge and the concavity. When a person with a protruding belly tightens the belt, the belly bulge becomes more noticeable, such is the situation. Other times, the skin is stretched by aging and simply wrinkles.

Major cosmetic surgeons remove the fat and insert the fat taken from the thigh into the orbicularis retaining ligament, but there is no need to do that. However, this is not necessary, because the protruding fat can be inserted directly into the orbicularis retaining ligament to solve the problem. This is the surgical method known as the hamula method.
The "back Hamula method" is to perform the Hamula procedure through an incision in the white of the back of the eyelid without cutting the skin.
If the skin wrinkles and sagging are strong, the skin is excised and the Hamula procedure is performed as it is, which is known as the "front Hamula" method.
In the front hamula, not only the skin is excised but also the orbicularis oculus muscle is incised to penetrate into the orbital fat, which is prone to paralysis and external rotation due to skin traction.
On the other hand, the back hamula procedure preserves these muscles, so the burden on the body is lighter and the downtime is shorter. Since the skin is not removed, the result is natural.
If the skin is removed, a scar is left under the eyelashes, which inevitably results in a somewhat artificial look.
However, the back hamula method has a very narrow surgical field and requires excellent technique. For this reason, there are very few doctors in Japan who can perform the procedure. The lower eyelid is structurally more directly subject to gravity than the upper eyelid and is therefore more susceptible to the effects of aging than the upper eyelid.
